Coaching Reimbursement Form
Coaches in good standing are eligible for reimbursement of a portion or all of their child's registration fee. Only valid for current year must be sent in before Sept 1 of current year.
In order for you reimbursement to be processed, please complete the form bellow.
If you coached more than one division, please complete multiple forms. This includes attendance, grading, PVSC, and coaches clinics.
Coaches MUST have provided a valid Police Vulnerable Sector Check (PVSC) in order to coach at TMSC and receive reimbursement.
